This form is a Microsoft Excel worksheet that must be downloaded, completed and sent to your school administrator as an email attachment. Your school administrator will then send the form to the board office for approval. You will be notified by email when your field trip has been approved by the director of schools. *You must fill out and submit the Nurse Field Trip Form when you submit the Field Trip Form Request.* Both are linked below.
